Update

I heard back from someone higher in our agency today. The birthmother only has to go to the orphanage, but they don't think she is close to it. They are looking into what they can do to help her.

The mistake was with a paper that she has to write in its entirety. It is not uncommon to find errors since it can be done under many different circumstances. My agency contact doesn't think it is a major problem, just an inconvenient delay. She seemed very encouraging and I do feel better.

She also sent us SIX new pictures!!! Same walker, winter hat, and Robeez like shoes. Also, the beginnings of 2 bottom teeth!
