The new form came and it was WRONG again!!
The requestor was my husband's name, but his middle name was wrong, as was the house # in our address. I asked the woman from the Secretary of State office if it was an issue since his name and SS# were correct lower on the sheet. She checked and came back to say that they wanted it perfectly the way it was, so they are going to do it again!
Since they were, I also asked if they could get another notary to certify it since the one they used in the State Police office's commission ends in July 2008 and it was recommended that we make sure there was at least another year left on it. She said OK, she would get someone who's expires in 2009 to do it and they will mail it out again!
They sent it again and it was finally correct.
